1. Do I Have Information on My Member Data Record (MDR), How Do I Update?
If there is any
necessary information in your record, just follow the following: A. Fill the
PhilHealth Member Registration Form or PMRF *. B. Submit the PMRF with
supporting documents to the nearest PhilHealth office or email to
ofp@philhealth.gov.ph. * Be sure to sign the PMRF as proof that all the
information provided is correct and true

To find out your PhilHealth Identification Number or PIN,
text on 0917-5129149 or email to ofp@philhealth.gov.ph the following
information: LAST NAME FIRST NAME MIDDLE NAME BIRTHDAY
3. How to Request Member Data Record (MDR)? What are the
Requirements?
Simply email your complete
details (Last name, Full name, Middle name) and date of birth at
ofp@philhealth.gov.ph and your copy of MDR will be received immediately. You
can also request a copy of it at any PhilHealth office in the country.
4. Will My Partner declare
our child even when we are not married?
Yes. Only submit a PMRF
where the child's name is written dependent and include the copy of his birth
certificate as proof of his / her child.
5. Can I Declare My Common Law Wife / Husband?
Only legal spouses can
be declared dependent apart from children under age 21.
6. I am a Voluntary Member and now OFW. What are the
Documentary Requirements for Updating My Membership Category?
To update your
membership category from voluntarily to OFW, fill in the PMRF and show one of the
following documents as proof that you are OFW active: • Valid Overseas
Employment Certificate (OEC) • Valid E-Receipt; • Valid Working Visa / Re-entry
Permit; • Valid Job Employment Contract; • Valid Company ID issued by employer
abroad; • Certificate of Employment for applicable period from employer abroad;
• Cash remittance receipt from member abroad at least 2 months prior to the
date of renewal / payment • Valid ID / Certificate issued to OFWs in the host
country (e.g. Iqama, HK ID, Soggiorno); • Any other equivalent proof, subject
to the approval of the authorized officer.
7. What
are the Benefits of Paying a Contribution?
If you pay a contribution, your need is your PhilHealth
Identification Number (PIN). Make sure your PIN is correct so other members
will not be able to record your premium.
8. Where Can I Verify My Contribution?
A. Your online payment
can be found by registering or logging into the Member Inquiry facility on
PHILHEALTH homepage.
B. The MDR is also the most recent date / duration of your
paid contribution.

for a HISTORY OF PAYMENT CONTRIBUTION at any of the nearest PhilHealth offices

9. I have Employee Insurance abroad, Do I still have to pay
PhilHealth?
Yes, because it is
mandatory for all OFWs. Even abroad, you can still avail PhilHealth benefits
other than insurance provided by your employer. It is also for your qualified
dependent (if any) covered in your PhilHealth
10. Can I
Pay the Months That I Can not Define?
OFWs like you are
given a one-month or 30-day grace period from the expiry date of validity
coverage to pay a person's contribution.
11. When Can I Use My Benefits?
If your contribution
is updated, you can use your benefit within the validity coverage indicated on
your MDR.
12. Is MDR still mandatory in Hospitals?
If the hospital has a
"My PhilHealth Portal," PhilHealth Benefit Eligibility Form (PBEF) is
sufficient to limit PhilHealth benefits to your hospital bill. MDR is required
for hospitals without "My PhilHealth Portal."
13. How Long is the Claim Reimbursement Process?
By law, PhilHealth has
60 days to process claim reimbursements with complete documents
14. Are there PhilHealth Staff at the Hospital that We Can
Ask About Our Benefits?
Yes. PhilHealth has
assigned registered nurses to selected hospitals to meet the needs of members
regarding their benefits. They are called PhilHealth CARES or Customer
Assistance, Relations and Empowerment Staff located in the admission or billing
section of hospitals accredited by PhilHealth in the Philippines.
