1. How to Register?
Fill out the PhilHealth Member Registration Form (PMRF). It
can be requested at any PhilHealth office or download here. Submit the PMRF
to the Human Resource Department (HRD) of your Manning Agency. You will receive
from your Manning Agency of PhilHealth ID and Member Data Record (MDR) which
holds a permanent PhilHealth Identification Number (PIN
2. How to Update PhilHealth Information?
If there is any required information in your record, just
follow the following.
A. Fill in the PhilHealth Member Registration Form or
PMRF.
B. Submit the PMRF with supporting documents to the
nearest PhilHealth office or email to ofp@philhealth.gov.ph.
* Be sure to sign the PMRF as proof that all the
information provided is correct and true.
3. Can I Continue to Contribute to PhilHealth Contribution at
the end of the Contract?
Yes. We encourage all our seaman to continue their
contribution to PhilHealth even if their ship's contract is over to ensure the
benefit of PhilHealth benefits. It is only necessary to update the membership
category of PhilHealth from the Formal Economy to the Informal Economy member
upon completion of the contract. The premium contribution amount payable as an
Informal Economy member is P600.00 per quarter and is payable to the nearest
PhilHealth office or to any accredited collecting agents throughout the
Philippines.
4. What are the Required Documents to Use the Benefits?
For local confinement:
- Fill out PhilHealth Claim
Form 1 (CF 1).
If the member is currently
on board the ship, or the contract at the time of the confinement has not yet
expired, make sure the Manning Agency signed Part 2 of the CF 1. CF 1 may be
requested from the hospital or otherwise downloaded here.
- MDR copy
- Proof of fundamental
contribution (if not stated in MDR)
For confinement abroad:
If you have been confine
abroad - submit the following with CF1 to any PhilHealth office in the
Philippines within 180 days of hospitalization:
- Statement of account
translated into English
- Copy of medical
certificate transcript in English where the final diagnosis, confinement period
and service performed
- Receipt of payment to
hospital and doctor.
